- Original and photocopy NBI Clearance - must be DFA Authenticated ( issued in current year)
- 1 pc.. - 2x2 colored picture (any background) attached in visa application form NOTE: DO NOT SUBMIT scanned pictures.

- Completely filled up WAIVER FORM of WWW Express (Affiliated with DHL)[waiver form]
